The influence of infusion therapy with a colloid-hyperosmolar solution on general trophic and detoxification processes in rats under conditions of burn shock

Abstract

The experimental investigation is devoted to the preclinical study of the therapeutic effect of a new colloid-hyperosmolar solution HAES-LX-5% in comparison with the reference preparation Lactoprotein with sorbitol (LPS) in the disturbance of the homeostasis of the rat organism in conditions of burn shock (BS). The effect of HAES-LX-5% on the dynamics of survival, body weight, systemic inflammatory response and endogenous intoxication was investigated.

At the first stage, the effect of HAES-LX-5% and LPS on general trophic and detoxification processes in rats without a burn was studied. It was found that HAES-LX-5% and LPS contributed to a significant increase (p < 0.05) in body weight of animals as compared to the baseline data and did not significantly affect the leukocyte intoxication index (LII). This indicates the absence of their negative effect on general trophic and detoxification processes in animals without burns. The low effectiveness of infusion therapy of rats with burn shock with the help of 0.9% NaCl solution (control pathology) was confirmed by the probable (p < 0.05) increase in mortality from 5.0% in the first 24 hours up to 11.0% in the interval from 96 to 120 hours. The treatment of rats with HAES-LX-5% and LPS significantly prevented the death of animals in the first 120 hours of the experiment.

Under the burn shock conditions, a significant (p < 0.05) lag in the physiological growth of the body weight of animals was established against the background of introduction of 0.9% NaCl solution, which indicates the inability of the drug to completely prevent the development of destructive and toxic processes. Under the influence of HAES-LX-5% and LPS, the body weight of animals in 71-158 hours practically reached the values of intact animals, and was significantly higher (p < 0.05) in relation to the initial data and the data against the background of the action of 0.9% NaCl solution.

To assess the degree of intoxication, LII, C-reactive protein, malonic dialdehyde (MDA) and the level of medium molecular peptides (MMP) in blood serum. In animals with BS against the background of 0.9% NaCl solution, the index of LII in 158 hours increased probably 5,37 times (p < 0.01) as compared to animals with a burn. Against the background of the introduction of HAES-LX-5% and LPS, the level of LII increased just 2,33 (p < 0.01) and 2,41(p < 0.05) times. In the animals of control pathology group, they noted an increase of the content of MMM in blood in the first 24 hours by 75,6%, in 71 hours – by 159,5%, and in 158 hours – by 128% respectively as compared to the intact animals. The pharmacologycal correction of BS with HAES-LX-5% and LPS solutions detained the development of endotoxicosis from 71 to 158 hours.

Thus, it has been experimentally proved that HAES-LX-5% reduces the level of endogenous intoxication, improving general tropic processes and improving the survival rate of rats. The harmlessness of the HAES-LX-5% in animals without pathology and the absence of potential allergenic properties peculiar of the LPS comparison preparation have been established.
