Експериментальна та клінічна фізіологія і біохіміяObjective: to investigate the balance of free-radical activity and antioxidant defense of the oral cavity for determining the efficacy of the local action of the drug «Tantum Verde» during the postoperative period.
Materials and Methods: Patients were divided into two groups in the performed investigations. The control group included 20 persons who have been administered traditional postoperative treatment.
30 patients of the main group were carried on the improved methods of the postoperative treatment, which, along with the above-mentioned scheme, topical application of NSAIDs «Tantum Verde» was suggested.
All patients underwent the study and the comparison of the balance of free radical activity and antioxidant defense of the oral cavity.
Results: The increase of MDA content in comparison with the norm index was marked in the control group on the next day after the surgical intervention. Maximum values of the investigated index in this group were detected on the third day which was prevailing norm index. The intense decrease of MDA content in the oral fluid was observed in the period before the 5th day, which is currently dominated by the initial values. At the end of the observation the imbalance of obtained data was subsequently more than the norm index in the control group.
Similarly, the growth of MDA concentration in the oral fluid to the same level as in the control group was marked in the patients of the main group on the first day. However, the decrease of this index in comparison to the previous period of the study was revealed on the third day in this group. The indicated decrease was noted further: on the fifth day, the obtained result was equal to that of the norm index, and on the seventh day its further decrease was revealed. The damaging influence of free radicals, which are permanently formed in the body, prevents the multistep antioxidant protection system. One of its components is the enzymatic link, catalase (CA) – the enzyme is its representative decomposing hydrogen peroxide created during biological oxidation, in water and molecular oxygen.
As the main function of catalase is the destruction of toxic hydrogen peroxide formed during the various oxidation processes in the body, it is necessary to find out the difference of its activity in the oral fluid of patients in both groups for determining the efficacy of local action of the drug «Tantum Verde» during the postoperative period.
Under physiological conditions, the processes of free radical formation occur permanently and under the control of multi-stage antioxidant defense system. As a consequence, a stable antioxidant-prooxidant balance is formed in the tissues.
Conclusions: The imbalance in the oral cavity with a shift in the pro-oxidant direction in the initial stages of the postoperative period is noticeable caused by the intensive formation of free radical compounds and the activation of lipid peroxidation and inhibition of the antioxidant defense system. The signs of normalization are marked in 7 days only. Local treatment with «Tantum Verde» allows to decrease the level of lipid peroxidation in the damaged tissues, and respectively, to reduce the load on the antioxidant defense system and to avoid its overloading.
As a result, it is possible to achieve physiological balance of these two systems on the fifth day after surgical treatment.
