Endovideosurgery intervention in pathology of upper urinary tract. Comparative characteristics of the patients

Анотація

The study was carried out during 2009–2013. Urology department at the Khmelnytskyi Regional Hospital and includes data obtained from 326 patients.

In order to conduct a comprehensive analysis of clinical data reviewed by the medical statistics department of urology and formed appropriate nosological sample of patients. A subgroup of patients with renal cysts included 130 patients (41,7 % of all surveyed) – 96 core group of EVH operations and 34 – control. By age in the subgroup of patients were 18 to 74 years (mean age 53,5 ± 1,49 years), by gender – 70 men and 60 women. Dimensions renal cystic formations ranged from 48 mm to 120 mm, the average size of the cysts was 68 ± 18 mm. Regarding the location - right sided cyst was diagnosed in 42 % of cases, left-sided – in 44 %, bilateral – in 14 %. Most cysts were located in the posterior segments of the kidneys.

A subgroup of patients with urolithiasis included 126 persons (44,9 % of all surveyed) – 86 patients of group operations and EVH 40 – control. By age were included in the subgroup of patients 18 to 77 years (mean age 54,1 ± 1,52 years), by gender – 66 men and 60 women. Stones were located on the right side in 44,1 % of patients and on the left in 55,9 % of patients. In 35,3 % of patients ureterolitotomia was performed through laparoscopic access in 64,7 % of patients ureterolitotomia was made using retroperytoneoscopic access. The average size of stones in patients at laparoscpopy of ureterolitotomy was 13,2 (± 3,2 mm), the group with retroperytoneous ureterolitotomy average size of the stone was 13,5 (± 4,19) mm.

Group of patients with renal tumors was minimal and included 56 people (13,5 % of all surveyed) – 36 patients from the operation group and EVH 20 – control. According to age the patients were devided into subgroup of patients 26 to 79 years (mean age 58,2 ± 2,62 years), according toy gender – 26 men and 30 women. When a kidney tumor was removed, diameter of the tumor was 5,5 ± 1,3 cm (from 4,0 to 8,5 cm). Left-sided localization was in 36 patients, right-sided in 20; tumor of the upper pole of the kidney was determined in 16 patients (28 % of the total), the lower pole of the kidney – in 19 cases (31 %), middle third – in 22 patients (41 %). In 44 patients (79 %) were found to have cancer of the kidney T1b stage, in 12 patients x – T2 stage. In all patients diagnosed with renal cell carcinoma was confirmed histologically. In patients who underwent nephrectomy EVH method was used laparoscopic (20 patients) and retroperytoneoscopic access (16 patients).

Diagnostic algorithm should be used in full, but should begin with non-invasive and inexpensive investigation. There was no correlation between communication characteristics of water, food, the presence of bad habits and susceptibility to the development of kidney stones, kidney cyst formation. Most cysts were located in the first and second segments of the kidney. In clinical examinations in 82% of patients dominated complaints of a dull pain in the lumbar arching plot. Clinical examination of 93,5 % of patients proved that urolithiasis was the main complaint of pain in the lumbar region.
